We aim to provide a person-centred approach to care, valuing independence, rights and choices when visiting our clients in their own homes.
We offer a variety of support services such as attending health care appointments, assisting with shopping, light housework, budgeting for meals and meal planning/cooking and general daily living. We will not assist with personal care tasks or administering medication, but prompts are within the scope of our roles. These are regulated activities which we are not able to complete.
We understand that everyone will live how they choose to and with the support and guidance from our team of experienced staff, we aim to encourage people to access their community, fulfil an active and inclusive life and achieve their goals.
Each person will have their own personalised care plan and risk assessments. This will be completed with the person and also with them in mind. We will personally match staff to the individual so that they are best suited to work with them and to ensure that relationships are formed. Trust and respect are key focuses on our ethos with the company.
Our experienced counsellors can be on hand to offer online sessions and guidance should anyone need more specific support.
We will organise an initial assessment to be completed in an environment of your choice, this could be in your home, in a hospital ward or clinical unit. We will work with you to find out about your personal information, background history, needs, wishes and support requirements. You are welcome to have a family member, friend or another professional with you, should you wish to. This may help you to share your personal information and also inform us of any knowledge that would support your care plan later on. We aim to build up a relationship with you from this initial meeting so that you feel confident in what support can provided, understand our ethos, terms and conditions of care and also the scope as to what can be provided.
We will go through some paperwork with you and we encourage you to ask any questions that you may have as this helps you to make an informed choice as to whether care and support is what you really want or need.
Each and every service user will have an individualised and unique care plan. This will be written with you, about you and will cover all your personal needs, wishes, background and preferences. A care plan is essential to ensure that you consistently receive the right level of care long term and that your personal requirements are known by care workers and professionals involved in your care.
We will sit with you to find out about what matters most to your life, how we can support you in the best possible way to retain personal identity, independence and participation.
An effective care plan will help you to better understand your condition, live as independently as possible and have more control over your life.
By being involved in your own care planning, it enables you to have a voice and share any changes that need to be made, when they need to be made and ensures that you will be looked after in the way that you want.
